I think it's mostly the fact that peoples were continuing working on that base instead of cleaning it at first. Having a more complex code on this crappy base (about the Quake 3 code left here and there), makes it even less practicable of work!
Cleaning the Quake 3 code would practically take a full diagnostic rewrite. It would take months with someone working full days on it. The appended code should be clean though and one of the more important gameplay files (bg_pmove.c) should be fairly competent.

I think it would be easier to spot the important parts/principles and remake a clean and reorganized code from them. Cleaning the actual code would be too hard and too long, even for an experimented coder..
Edit: Oh, and.. what the love*** IS THIS FREAKING FILE ??!
HOW IS IT POSSIBLE TO MAKE SUCH A THING ???????????!!!!!!
