I think you mean big-budget commercial games. Surely someone with a desire for games of the type you have described is an avid follower of the indie gaming scene, no?
Look no farther than Super Meat Boy if really want a more modern example of a game with a challenging difficulty curve not based on number scaling or AI reaction handicapping. No, there are no game over screens or x amount of lives, but that hardly removes from the difficulty factor on a stage-basis later on.
Aquaria, Bastion, Braid, Cave Story, Gish, Iconoclasts, Night Sky, VVVVVV, Within a Deep Forest. All games in the last decade that offer challenges based on player reflexes and/or critical thinking without overly pampering the experience.
yeah I'm only referring to big budget companies, since that's apparently what the topic conversation was heading to.
As for indie games. I just went though antichamber and it was pure awesome. Truly a work of art. I also finished Cave Story with the exception of the "heck Run" as that thing is hard as ... well heck :s
The difficulty spike in that area is insane compared to the rest of the game. But mark my words I will finish even that once :p

Hitman is not hard or discomforting. All you need is good observation skills. The difficulty comes down to pattern memorization rather than pure reflex. Sure there are situations where you are put in a situation where you have to react fast, but in the end its all scripted with a way out presented to you on a golden plate.
Tell me when was the last time you got a "Game Over" screen in a game and were forced to restart from scratch.
You could say playing x-com on iron-man mode could fit the description, but the most difficulty in that game stands from odds stacked either in your or the AIs favor.
Having game saves is nice, but having auto saves happen every 5 minutes to make sure the player never gets frustrated with a game is more discomforting than the crappy AI and false difficulty of stacking the odds in the AIs favor.

But to me all that makes is boredom. And just increasing enemy HP and attack power is nothing more than stacking odds which make for fake difficulty.
How about behavior patterns, make them smarter, make them move differently, react faster, change their tactics and not just buff up their stats. that's a sign of sloppy game design.
So tell me. When was the last time you felt REALLY proud for beating a game with fair odds?
Game overs are good things, iron-man is a good thing. Not being able to skip though videos is a bad thing as well as gameplay that restricts the player the the story and killing him for doing anything that isn't by the story script ^^;

This may be personal preference, but replaying Ninja Gaidens last stage 10 times over because of the final boss gauntlet was never discomforting nor got boring. Having to sit though the same cut scene over and over in Rainbow six Vegas is. And I played all the hitman games except Absolution on which I saw quite a few videos. And quite honestly. Hitman 2 was much harder (though probably due to some discomforting bugs).
But that just shows if the game is designed correctly, having to replay something is not boring. It only becomes boring if the gameplay is lacking that something to make it interesting.

watching videos does not do hitman absolution justice , its a true hitman game , hitman 2 was not that hard , try it
