You'll need one or more of the following :

  • A model editor [Milkshape, 3dsmax, Maya, Lightwave, Blender, XSI, etc.]
  • A map editor [GTKRadiant, QERadient, NetRadiant, etc.]

The idea is simple. You will model what you want in your model editor, import into your map editor, then compile. Step by step :

  • Create model
  • UV map model
  • Texture model
  • Export model as .obj/.ase
  • Import model into Map Editor
  • Assign shader/texture to selected model
  • Repeat above steps for each model
  • Add a skybox to your map
  • Compile without -vis or -light
  • Enjoy!

That's just a rough summary as there may be several other tiny in-between steps, but it should give you a generalized idea.
